Abstract:We aim to analyze the characteristics of infections in pemphigus including localization and pathogen spectrum, to explore the risk factors of infections, and to provide guidance for clinical treatment and prevention. This retrospective case-control study enrolled 142 pemphigus patients hospitalized in Huashan Hospital from January 2017 to December 2021 and information including infection rate, susceptible location and pathogen distribution was collected through medical records and laboratory results. Univariate analysis and multivariate logistic regression analysis were performed to identify the risk factors of infections. 74 (52.1%) of 142 hospitalized pemphigus patients had infection. Univariate analysis of risk factors showed that infected group patients had more hypoalbuminemia (P < 0.001), used more systemic glucocorticoid treatments (P <0.001) and used higher cumulative dose of gluco-corticoids in recent 3 months (P=0.001). Multivariate logistic regression analysis showed that hypoal-buminemia (OR=7.90,95% CI 1.92-32.49) and systemic glucocorticoid treatment (OR=6.25,95% CI 2.63-14.83) were risk factors of infections in pemphigus patients. The most susceptible locations of infection were wound surface and lung, and most common pathogens were Staphylococcus aureus, Pseudomonas aeruginosa. In conclusion,hypoalbuminemia and systemic glucocorticoid treatment were risk factors of infections in pemphigus. Related measures are supposed to be taken as early as possible to prevent and control infections.
