胫骨平台骨折患者伤口分泌物中分离出肠炎沙门菌1株

A Salmonella enteritidis strain isolated from the wound secretion of a patient with tibial plateau fracture

摘要

海南医学院第一附属医院临床微生物实验室于2022年6月16日从左胫骨平台骨折患者伤口分泌物中分离培养出1株革兰阴性杆菌,经全自动微生物鉴定药敏分析系统、血清凝集和基质辅助激光解吸飞行时间质谱等鉴定为肠炎沙门菌。根据病原学检测及药敏试验结果,序贯给予该患者美洛培南1.0 g q8h静脉点滴9 d,哌拉西林钠/他唑巴坦钠4.5 g q6h静脉点滴5 d,美洛培南1.0 g q8h静脉点滴19 d,万古霉素1.0 g q12h静脉点滴10 d,以及左胫骨平台感染清创+引流术后,患者伤口处未见明显渗血渗液且感染指标未见明显异常,最终治愈出院。

Abstract

The clinical microbiology laboratory of the First Affiliated Hospital of Hainan Medical University isolated and cultured a Gram-negative bacillus strain from the wound secretion of a patient with left tibial plateau fracture on June 16, 2022, which was confirmed to be Salmonella entrica subsp. enterica serotype enteritidis by automatic microbial identification system and matrix-assisted laser desorption / ionization time of flight mass spectrometry(MALDI TOF-MS)experiments. The patient was treated with meropenem 1.0 g q8h intravenous drip for 9 d. According to the results of etiological detection and drug susceptible test, the following medication regimen was adopted: piperacillin tazobactam sodium 4.5 g q6h intravenous drip for 5 d, meropenem 1.0 g q8h intravenous drip for 19 d, vancomycin 1.0 g q12h intravenous drip for 10 d, and debridement and drainage of left tibial plateau infection, the patient was cured and discharged.
